Sign in with mobile number: Tap “Sign in” then choose “Sign in via mobile number.” Enter your phone number and tap “Get code,” followed by “OK.” Finally, input the verification code received from the message.
Sign in with username and password: Tap “Sign in”, then choose “Sign in via password.” Enter your username and password and tap "Sign in."
Sign out: Tap “Profile” on the bottom right corner of the home screen, tap your username, then scroll down and select "Sign out."
1. Plug in & cuff up: Insert the cuff tube firmly into the side of the device. Wrap the cuff on your left arm, about ½ inch above the elbow. It should be snug – two fingertips should fit under the top edge. Rest your arm on a table at heart level.
2. Connect in the app: Open the app and tap “Blood Pressure Start.” Wait for “Device connected!” before proceeding.
3. Take the reading: Tap “Start Measurement.” Stay relaxed, don’t talk, and let the cuff inflate/deflate. If the cuff feels too tight, tap “Stop.”
4. Log medications: When prompted, confirm whether you’ve taken blood pressure medication earlier.
5. You’re done: Results automatically save to your account and are sent to your Care Team. For step-by-step instructions on taking a measurement with your Ease blood pressure monitor, please refer to the Quick Start Guide for your device. It walks you through proper setup, positioning, and how to begin a reading using the app.
1. Prepare the lancing device: Wash and dry your hands. Insert a new lancet into the lancing device and adjust the depth if needed.
2. Insert a test strip: Take one strip from the vial and immediately close the cap. Insert the strip into the BG5S meter. The meter will turn on and show a “blood drop” symbol, meaning it’s ready.
3. Get a blood sample: Use the lancing device to prick the side of your fingertip. Gently squeeze to form a small drop of blood.
4. Apply blood to the strip: Touch the blood drop to the absorbent edge of the strip. The strip will draw in the blood automatically. Hold until the meter beeps or begins counting down.
5. Read your result: In a few seconds, your blood glucose result will appear on the screen. The result is automatically stored in the meter/app.
6. Finish up: Safely dispose of the lancet and test strip in a sharps container. It will walk you through proper setup, positioning, and how to take a reading using the app.
"Tips” will give you helpful guidance to take more accurate blood sugar readings.
“Tutorial” will walk you through each step, from gathering your supplies to preparing the lancet to taking a reading.
Tap the "Plus (+)" button, then select “Food Diary.” Take a picture of your food, choose a meal, and optionally add notes. Finally, tap the “Submit" button to complete the process.
Food Diary review: Your care team will typically review submitted food diaries within 4 business hours. Response time may vary depending on volume of requests and time of day.
Please make sure you have allowed access for Unified Care to use the camera:
For iOS users: Go to your device settings, choose Unified Care, and then turn on the camera.
For Android users: Go to "Settings" → Tap "Apps & Notifications" → Choose "Unified Care" → Tap "Permissions" → Select "Camera" → Tap "Allow."
To view your past measurements in the app: Tap “History” on the bottom menu. You'll see a list of past readings organized by date. Tap any entry to view full details, including measurement time and trends. You can also explore charts or export data to share with others.
Connect a health app to your Unified Care app: Tap “Profile” on the bottom right corner of the home screen → Tap "Settings" → Scroll down and tap "Linked health apps" → Connect Apple Health for iOS or a native health tracking app like Google Fit for Android.
Allow Unified Care to access walking data:
For an iOS (Apple) device: Go to Settings → Health → Data Access & Devices → Unified Care → Enable “Steps”
For an Android device: Go to Settings → Apps & Notifications → Unified Care → Permissions → Physical Activity → Allow
To check your exercise progress in the app:
Tap "History" at the bottom of the home screen and then scroll down to view "Exercise" data

To save an article:
1. Open the article.
2. Tap the bookmark icon labeled “Save” at the bottom left.
To view saved articles:
1. Go to the Explore tab.
2. Tap the bookmark icon in the top right.
3. Your saved articles will appear here.
To share an article:
1. Open the article.
2. Tap the “Share” button at the bottom left.
3. In the popup that appears, choose how you’d like to share.
To enlarge an article's font size: On the Explore page, choose an article and then tap "AA Font Size" on the bottom right side of the screen to increase the font size.
To select a different language in your app:
1. Tap “Profile” on the bottom right corner of the home screen.
2. Scroll down and select “Languages.”
3. Choose your preferred language from the list.
4. Tap “Yes” to confirm.
5. Your app will refresh automatically, and the language will be updated.
To add a new device:
1. Open the Unified Care app and tap “Profile”, then select “My Devices.”
2. Tap “Add New Device”, choose “iHealth Gluco+” (or another supported device from the list).
3. Tap “Scan QR Code” and use your phone’s camera to scan the QR code located on the bottom of your device.
4. Once the QR code is recognized, tap “Add Device” to finish pairing.
Check that you have allowed access for Unified Care to use the camera:
For iOS users: Go to your device settings, choose Unified Care, and then turn on the camera.
For Android users: Go to "Settings" → Tap "Apps & Notifications" → Choose "Unified Care" → Tap "Permissions" → Select "Camera" → Tap "Allow."
You can easily order more test strips in the Unified Care app:
1. Tap “Profile” on the bottom right corner of the home screen.
2. Choose “Order Supplies”; the default supply refill is 200 lancets and 200 test strips.
3. Tap "Next" and enter your shipping info.
4. Once you enter all required information, tap the “Submit” button to place your order.
