1. Bluetooth Connected Devices: Each patient enrolled in Unified Care can receive up to four Bluetooth connected monitoring devices. This can include the iHealth Ease Blood Pressure Monitor, iHealth Nexus Wireless Body Composition Scale, iHealth Smart Glucometer, and iHealth Air Pulse Oximeter. All of our devices are manufactured in-house.
2. Software Platform: Providers can login to Unified Care’s data-driven platform or provider app to view patient data and interact with clinicians and patients.The software includes a smart alerting system, visualized patient data, and triage features.
3. Clinical Care Team: Unified Care’s trusted, multilingual care team includes Registered Dietitians (RDs), Certified Diabetes Educators (CDEs), and Clinical Assistants (CAs). A care team is assigned to each clinic depending on the size and needs of the patient population at your practice. Care team members monitor patients 12 hours a day, 7 days a week.
4. Patient App: Unified Care’s user-friendly mobile app is available for every enrolled patient. The app connects with the patient’s Bluetooth devices and includes interactive tutorials and goal setting, among other features.
5. Care Coordination: Unified Care’s care team uses the software platform to escalate issues to providers on an as needed basis.

Patient Eligibility:
Patient Enrollment Consent: To enroll in the program, patients first give verbal consent to their provider. Then, a Unified Care clinician will ensure that a consent form has been obtained electronically through the patient app.
Manual Data Entry: No, the Unified Care app does not have an option for manual entry of patient data, per CMS guidelines for RPM. All the vitals are automatically updated in the app via Bluetooth connected devices.
Data Reporting and Insights: As part of the Unified Care program, providers will receive clinical outcomes data in addition to monthly billing reports that can be used to submit claims for reimbursement. Reporting can be tailored to a health care center’s needs.
