Have you ever looked at a high or low blood sugar (BG) or blood pressure (BP) reading and thought, “What caused that?” That’s exactly why the “Notes for Myself” feature exists in your Unified Care app. Even a quick note can help connect your habits to your health numbers—for both you and your care team.
Where to Find the “Notes for Myself” Feature
- Go to History
- Tap “View more” next to the vital you want to review

What Kind of Readings Should I Leave Notes On?
It’s up to you—but the more notes you leave, the more you help your care team and yourself understand your patterns and add context.
🔴 High:
“Didn’t sleep well”
“Ate pizza and dessert”
“Skipped lunch, overate dinner”
“Stressful workday”
🟡 Low:
“Skipped breakfast”
“Exercised without a snack”
“Took meds but didn’t eat”
“Felt dizzy”
🟢 In Range:
“Slept well and ate a good breakfast”
“Walking daily”
“Started strength training”
“Eating more protein”
“Back on routine after travel”
Why It Matters
Notes help you reflect on helpful or harmful habits, give your care team insight between visits, and reveal patterns—like higher BP on Mondays or BG dips after late dinners.

✅ A short phrase is enough.
